Primary Schools -
A. Testing ( 3 Cluster
Assessment Tools - see below ) for the Transition Report will
need to be completed by teachers for Grade 6 students.
The Whole Transition Report will only be filled in for those students
who are attending cluster schools at a secondary college level
and teachers will need to update this document and other relevant information
relating to the testing and pass this onto relevant secondary
schools before the last transition visit at a secondary school
for the year. Testing sheets completed by students can be put into student files
recording student time at a primary school but they must also be
attached to the back of the Transition Report. In 2006 and 2007
testing will occur in each semester. The Transition Report
Test Results must be sent to the Educator via mail also, so that
Cluster Data collation can occur at the end of the year. This is for
all Grade 6 students !!!
* If students are not attending
Cluster Secondary Colleges then other schools may want different
transition documents implemented. This is the responsibility of the
schools, but cluster data relating to the 3 common tools is still
needed !!
B. Student Passports will be completed by students in grade 6 either electronically or
written depending on the resources of the school. These will
also be completed at the end of the year, collected by teachers
and passed onto relevant secondary schools before the last
transition visit.
Secondary Schools -
All Secondary Schools are required
to complete the 3 Cluster Assessment Tools in 2006 and beyond. The Internal collation of information is the responsibility of the
school, so no information will need to be recorded on the Transition
Report as this is only used for grade 6 students for transition
purposes. However, all Student Testing needs to be send via mail to
the Educator so that Cluster Data can be collated. This should occur
as testing occurs !!
